MEDICAL RELEASE STATEMENT: I, as parent or guardian of the participant, give permission to Mount Snow Ski Patrol or any other medical personnel to treat named participant in the event of a medical emergency or illness. I understand that every attempt will be made to reach me as soon as possible.

CODE OF CONDUCT: The following is a code of conduct that is expected to be followed by all Mount Snow Outdoor Exploration Camp participants (“campers”). Campers agree to demonstrate appropriate, sportsmanlike conduct through the following behaviors: campers will behave in a respectful and responsible manner. Campers will show respect to their counselors, camp staff, Mount Snow employees, and the general public. Respectful and responsible behavior includes respect for property, courtesy and good manners, the use of appropriate language at all times. Campers will demonstrate respect for personal safety and the safety of others. Campers shall follow the directions of staff members.
Participants and participant parents/guardians understand the following actions or behaviors are prohibited:
•Actions which intentionally endanger the health and safety of themselves, other campers, and/or staff.
•Performing acts of bullying, aggression, or violence.
•Stealing, damaging, or failing to care for Mount Snow property, staff property, and/or property of other campers.
•Actions which continually disrupt daily programs.
•Using profanity or inappropriate language or displaying clothing or other personal items with offensive content.
•Performing inappropriate physical contact.
•Possession or use of illegal substances, tobacco, or alcohol.
•Refusal to follow behavior guidelines.
If a camper's behavior at any time threatens the immediate safety of him/her, other campers, or staff, the parent/guardian will be notified and expected to pick-up the child immediately.
Participants and participant parents/guardians agree that failure to comply with any of the above provisions can lead to disciplinary action which can be taken by the counselor or camp supervisor/manager. Any disciplinary action will be applied fairly and equitably to all parties involved. Failure to comply with the Code of Conduct may result in the removal from activities, the program, and/or additional fees. There will be no refunds given for campers who are removed from the program due to violations of the Code of Conduct. Nothing in this code shall be deemed to restrict the individual freedom of a participant while not representing Mount Snow.
PHOTOGRAPHIC RELEASE: The undersigned parent/guardian hereby grants, releases, and quitclaim to and Mount Snow the right and authority to use, sell, reproduce, and distribute the minor Participants photographs, likeness, recorded voice, or videotaped filmed appearance for promotional purposes or programs as they in their sole discretion shall deem appropriate. The undersigned hereby promises to make no claim or cause of action related to participant's likeness by Mount Snow.
